The bureau includes numerous programs such as: the Children with Medical Handicaps Program (CMH), the Ohio Hearing Aid Assistance Program, the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC), the Asthma Program, the Sickle Cell Program, the Infant Hearing Program, the Youth Homelessness Program, School Based Health Care Centers, the School Hearing and Vision Program, and many more.

Medical home care coordinators, primary/specialty care clinicians, service providers, and parent partners can contact the Bureau of Child and Family Health and/or the CMH program for additional resources and help supporting families in challenging situations. To be enrolled as a provider, an applicant must:
  • Complete the CMH Provider Application Form.
  • Complete the Internal Revenue Service (IRS) W-9 Form.
  • Document professional training and experience in treating children with handicapping conditions.
  • Provide verification of credentials.
  • Sign the CMH Provider Agreement.
  • Participate as a provider for the Ohio Department of Medicaid (ODM) Medicaid program. This requirement is waived for health departments, service coordinators, community dietitians, certain individual therapists and pharmacists.
